Skilled Nursing
Admission to the agency can only be made under the direction of a physician and is based upon a patients identified care needs.
Skilled nursing is provided by an RN or LPN with training and experience in providing care in the home.
The nurse communicates frequently with your physician to update the patient’s plan of care. Services may include:
- Physical assessments with routine vital sign assessments
- Education of patient, family members and caregivers on disease processes, self-care techniques and prevention strategies
- Wound Care
- Chronic Care Management (i.e. Congestive Heart Failure, Diabetes, Hypertension, Chronic Obstructive Pulmonary Disease, CVA)
- Medication reconciliation and Medication Management
- Feeding tube care and education
- Lab work to monitor conditions or high-risk medication dosage changes
- Ventilator and Tracheostomy Education and Management
- Ostomy Education and management
- Catheter Care
- Bowel Program
- Foot Care
Physical Therapy Services
Therapy Care must be under the direction of a physician. Physical Therapy services are provided by a licensed therapist or licensed therapy assistant under the direction of the therapist.
The advantages of receiving physical therapy at home is to maximize the patient’s ability to safely master the activities of daily living.
The Therapist communicates frequently with the physician to update the patients plan of care. Services may include:
- Therapy for aftercare from trauma such as a fracture
- Post-surgical conditions like a total hip or total knee replacement
- Patients having difficulty with mobility due to a diagnosis such as Parkinson’s, Multiple sclerosis, ALS, Heart attack, or Stroke
- Rehab due to frequent slips, trips or falls.
Speech Therapy Services
Speech Therapy services are provided by a licensed Speech Pathologist
- Speech Therapy pathologists work with the patient’s oral and motor cognition to help improve the patient’s swallowing, mental processing and language skills
Occupational Therapy Services
Occupational Therapy Care must be under the direction of a physician. Occupational Therapy services are provided by a licensed therapist or Certified Occupational therapy assistant under the direction of the Occupational therapist.
Occupational therapy can come in to assist the patient after nursing, Physical therapy or Speech Therapy have initiated the start of home health care.
The advantages of receiving Occupational therapy at home is to maximize the patient’s ability to safely master the fine motor activities of daily living that limit the patient’s daily functionality.
The Occupational Therapist communicates frequently with the physician to update the patient’s plan of care. Services may include:
- Therapy to use compensatory strategies for vision, sensory or motor loss that interfere with activities of daily living
- Strategies to conserve energy and reduce the demands of activities of patients with congestive heart failure or COPD
- Therapy to educate caregivers in their support with patients that have Cognitive and behavioral health conditions
Medical Social Work Services
- This service can be provided only after the need for Nursing or Therapy has been determined necessary.
Home Health Aide Services
- This service can be provided only after the need for Nursing or Therapy has been determined necessary
- Our trained Home Health aides assist with the patient’s daily activities of living such as bathing, eating, grooming and taking vital signs
